Life happens. People sometimes miss appointments, move house, change clinics or step away from HIV care for lots of reasons. If that’s you, you’re not alone – and you’re not in trouble.
Why this matters?
Staying connected to HIV care helps keep you well in the long term.
This page is here for people who:
- haven’t seen their HIV clinic for a while,
- have stopped treatment or are thinking about it,
- feel worried or unsure about going back.
The aim is simple: to help local services understand what’s getting in the way, and to support people to reconnect safely.
About this form
The short form linked below is hosted by our local HIV / public health partners. It asks about things like:
- how long you’ve been away from care,
- what has made it hard to stay connected,
- what kind of support might help you come back.
Your answers can help services:
- understand what people are facing in real life,
- look at how to make care more flexible and welcoming,
- identify where extra support might be useful.
The form is not for medical emergencies and does not replace speaking to your HIV clinic directly. You’ll see more detail about how your information is used on the form itself.

What happens after you submit
After you submit the form:
- Your responses go to the local team responsible for the project.
- They may use the information to understand patterns and improve services.
- If you give permission and contact details, someone may get in touch to offer support or talk about next steps.
